Please fill in all this fields, especially your allergies, medications, and family physician or referring physician (whichever is applicable).

If there is someone you would like to have access to your records, please print their name in the space provided at the bottom of the form and sign and date to confirm your permission.
Upon your arrival, we will also ask you to sign an Insurance Authorization Form, our promise to you that we will bill your insurance company before we bill you. As this form is generated patient by patient, it is not available for download, but will only require one signature upon your arrival.
